Alix Horton is licensed in Texas as a Licensed Master Social Worker (LMSW), license #120516. As an LMSW, Alix practices under the clinical supervision of Andy Davis, LCSW-S, a Council-approved clinical supervisor.

  • Andy Davis holds ultimate clinical responsibility for the services Alix provides, and regularly reviews her cases to ensure quality of care.
  • Client information may be discussed with Andy Davis as part of routine clinical supervision, consistent with the confidentiality protections described in Polaris Therapy, LLC’s Notice of Privacy Practices.

No Surprises Act

You have the right to receive a Good Faith Estimate of what your services may cost. An estimate of what your care will cost, given to you before it starts. The estimate is issued by Polaris Therapy, LLC. More detail is on the federal No Surprises Act notice (opens in a new tab).

Your rights as a consumer

Texas House Bill 4224 (89th Regular Session) requires this practice to post clear instructions for the following.

To request your health care records, sign in to the client portal and complete a records request form. Polaris Therapy, LLC holds the clinical record and responds to requests.

To contact the licensing authority, the Texas Behavioral Health Executive Council maintains a contact page (opens in a new tab), and its complaint process (opens in a new tab) explains how to raise a concern about a licensee.

To file a consumer complaint, the Texas Attorney General’s Consumer Protection division (opens in a new tab) accepts complaints online.
